The first link (PDF) works best if you need to print a form and fill it out, or if you need to give it to someone else to fill out. Use this choice if you are joining CCOBH or renewing your membership and need to mail us a check.

Chose the second link if you prefer to fill in the form ONLINE, print it out and mail it to us. This choice works best when you just need to tell us about changes in your organization's contact information.

Either form can be printed out, so whether joining CCOBH for the first time or renewing your membership, mail your completed form, along with a check payable to "CCOBH" to:

Membership is only $25 per year. To help support our efforts, we also request an additional contribution of $1 for each household in your organization. Smaller contributions are also appreciated. Memberships paid during the current fiscal year (1 July - 30 June) remain current until the end of the following fiscal year.
